ISU North American Adult Championships

Is anyone else here going? Maybe you can help me with the sign up. I am so confused!  :-[

I just signed up for this event and completed the Register for Individual Event link. However, in the Announcement, it says that a lot of information has to be sent e-mail in additon to completing this link. https://static1.squarespace.com/static/5c754a043560c34b3b67d9e6/t/5d2f3d18177d730001a4acf1/1563376921209/ISUATD19+Announcement.pdf The Announcement mentions a Music form, PPC form, Declaration form, and ID that must be e-mailed. No link to any e-mail except the contact e-mail at Lake Placid (so I assume that is where), and no link to the forms except on other ISU Adult Registration Pages (ie Oberstdorf etc.)
